Persistent w-sitting can cause (1) problems with decreased stability in the trunk and hips, (2) orthopedic issues with the hips, knees and feet, (3) and/or tightness or contractures in the hamstrings, hip adductors, internal rotators, and heel cords.
Who should not w-sit? For many children, W-sitting should always be discouraged. This position is contraindicated (and could be detrimental) for a child if one of the following exists:
There are orthopedic concerns. W-sitting can predispose a child to hip dislocation, so if there is a history of hip dysplasia, or a concern has been raised in the past, this position should be avoided.
If there is muscle tightness, W-sitting will aggravate it. This position places the hamstrings, hip adductors, internal rotators and heel cords in an extremely shortened range. If a child is prone to tightness or contractures, encourage anther pattern of sitting.
There are neurologic concerns/developmental delays. If a child has increased muscle tone (hypertonia, spasticity), W-sitting will feed into the abnormal patterns of movement trying to be avoided (by direction of the child’s therapist). Using other sitting postures will aid in the development of more desirable movement patterns.
During normal development, babies sometimes briefly move in and out of W-Sitting when transitioning from one position to another and this is normal. The problem occurs when the child remains in that position for an extended period of time such as during floor play or while watching television.

I had a patient recently ask me if it is okay for her thirteen-month-old daughter to be W-sitting during playtime. The short answer is: absolutely NOT!
It is never ok to be sitting like this for a prolonged period of time, and should be stopped immediately whenever you see it. There are a few postural and developmental reasons for this, and I'll go into some detail on all of them:
Hip Distortion
Knee and Foot Distortion
Poor Development of Core Muscles
Lack of Cross-Body Coordination
